Itemized Deductions:
- (Estimate) of out of pocket medical costs
- 1098 – from mortgage company
- Real estate taxes paid
- Charitable contributions – total contributed during the year
Other Deductions:
- IRA contributions
- Are you a teacher?
- Child care credit – please provide the following:
- Name of institution/address/EIN #
- Which child
- Amount paid
- Education credits – form 1098-T
Estimated tax payments:
- Any estimated tax payments that were paid during the year for current
- year taxes - please provide some confirmation of payments.
Copy of Drivers License:
- Copy of drivers license (front and back) for the taxpayer and spouse
Voided Check:
- bank information – for direct deposit of refund/direct payment
- Bank Name/Routing number/Account number
